Neapolitan Mastiff Ear Infections: Signs, Causes and Ear Care

Living with a Neapolitan Mastiff means living with a lot of dog: a giant guardian draped in loose skin, deep facial wrinkles and a heavy dewlap, with a short coat in gray, black, mahogany or tawny. Neos are devoted and usually calm at home, but they come with practical chores, from drool towels to fold cleaning. Their ears belong on that list, because the same loose skin and moisture that define the breed also affect the ear.

This article explains why Neapolitan Mastiff ears are prone to irritation, what early trouble looks like on such a wrinkled head, the common causes, when to involve your vet, and how to clean the ears of a giant dog safely.

Why a Neo's ears stay warm and damp

Many Neapolitan Mastiffs in the US and UK keep their ears uncropped. The uncropped ear is medium-sized, triangular, set above the cheekbones and hangs flat and close to the cheeks. On a head covered in folds, that ear sits in a very humid neighborhood.

  • Drop ears against heavy skin. The flap lies over the canal opening and presses against the wrinkled skin of the cheek, so there is little room for air to circulate underneath.
  • Drool and dewlap moisture. Neos drool, especially after drinking, eating or exercise. Saliva spreads across the lips, jowls and dewlap and can reach the base of the ear when the dog shakes or lies down.
  • Skin folds that hold warmth. Wrinkles on the face and neck trap heat, moisture and debris. Skin-fold irritation can sit right next to the ear and make the whole area itchy.
  • A drop-ear, heavy-headed build. UK primary-care research found that dogs with pendulous drop ears had higher odds of ear inflammation than dogs with upright ears. A Neo fits that profile.
  • Allergic skin. Large, wrinkled breeds can be prone to skin allergies, and itchy, inflamed ears are a frequent sign.

Size also matters in a different way: handling a Neo's head for a check is a two-handed job, and an uncomfortable giant dog can be hard to examine. Building calm ear handling into daily life makes everything easier.

Neapolitan Mastiff versus other mastiffs

The English Mastiff shares the breed's size and drop ears but has tighter skin and fewer folds; our Mastiff ear guide covers that dog. The Cane Corso, the Neo's fellow Italian guardian, is leaner and much tighter-skinned. The Neo stands out for the sheer amount of loose skin, wrinkling and drool around its ears.

Signs to look for on a wrinkled head

On a dog with this much skin, the first clues may come from the folds around the ear as much as the ear itself. Watch for:

  • Heavy, slapping head shakes that send drool flying more often than usual
  • Rubbing the head along the floor, a wall or your legs
  • Red, moist or smelly skin in the folds just below or in front of the ear
  • Brown or yellow waxy debris inside the flap
  • A sour or yeasty odor that is stronger than the usual drool smell
  • Holding one ear lower or tilting the head to one side
  • A thick, warm, swollen ear flap after a bout of shaking

Separating ear odor from drool odor can be tricky with this breed. Our article on why dogs' ears stink can help you think through what you are smelling.

Usual causes in Neos

Moisture from drool and folds. A constantly damp environment around the ear lets normal skin yeast and bacteria overgrow.

Fold irritation spreading. Inflamed skin in the facial wrinkles can extend toward the ear base.

Allergies. Environmental and food allergies often show up as itchy ears, paws and folds together.

Aural hematomas. Heavy ears and powerful head shaking can rupture small blood vessels in the flap, causing a soft, fluid-filled swelling that needs a vet.

Debris. Dirt, grass and bedding fibers can stick to damp skin and work under the flap.

When to see your vet

Cleaning is only for ears that look and smell normal. If something seems wrong, your vet needs to examine the canal and eardrum first. Call the clinic if you notice:

  • Pain: your Neo yelps, flinches or will not let you touch the ear
  • Head tilt, loss of balance, circling or unusual eye movements
  • Bleeding, or a swollen, puffy ear flap (this can be a blood blister called an aural hematoma)
  • A bad smell together with discharge
  • Redness or swelling that is getting worse
  • Symptoms that have not improved after a few days, or keep coming back

Ask your vet how to manage the skin folds near the ears as part of the same plan. Keep hydrogen peroxide, vinegar, alcohol and homemade oil mixtures away from the ears, and never push cotton swabs into the canal.

Ear care for a giant, drooly guardian

For most healthy Neapolitan Mastiffs, a daily wipe of the jowls and folds, a quick ear check a few times a week and a full ear clean about once a week work well. Plan around your dog's size: work on the floor, with the dog lying down or sitting against a wall, rather than trying to lift it.

  1. Wipe away drool first. Clean and dry the lips, jowls and dewlap so saliva is not carried into the ear.
  2. Clean the nearby folds. Gently wipe and dry the wrinkles in front of and below each ear.
  3. Lift the flap and check. Fold the triangular ear back and look inside. Stop and contact your vet if you see redness, swelling, discharge or a strong smell.
  4. Fill and massage. Fill the ear canal with a dog ear cleaner and massage the base of the ear for 60 to 90 seconds.
  5. Step back for the shake. A Neo's shake throws liquid and drool a long way, so stand clear or do this outdoors.
  6. Wipe and dry. Use cotton or gauze on the inner flap and the opening only, then dry the ear and surrounding skin.

Neapolitan Mastiff ear FAQ

Does drool really affect my Neo's ears?

It can. Saliva keeps the skin around the jowls and ear base damp. Wiping the mouth and folds dry after meals and water breaks is one of the simplest habits for this breed.

My Neo's ear flap suddenly swelled up like a pillow. What is it?

That is often an aural hematoma, a blood-filled swelling caused by hard head shaking. It needs a vet visit, and your vet will also look for the reason your dog was shaking.

Are cropped ears easier to care for than uncropped ones?

Ear cropping is a cosmetic choice and is restricted or banned in many countries. Uncropped ears need regular checking because they cover the canal, but a consistent routine keeps them manageable.

How do I check the ears of a dog this big without a struggle?

Practice short, calm handling from puppyhood, work with the dog lying down, and use rewards. If your Neo resists more than usual, an ear may be sore, so let your vet look.
